Requesting a file attachment on a form

When a form needs a document — a signed waiver, a resume, a certificate of insurance, a W-9 — add a File attachment field so the person uploads it right on the form.

Adding the field:
1. Open the form and add a field of type "File attachment".
2. Give it a clear label ("Upload your certificate of insurance") and mark it required if the document is mandatory.
3. Publish the form.

What the person sees:
A file picker that accepts common documents and images (PDF, Word, images, and more), up to 15 MB per file. After they choose a file it uploads with a progress indicator and then shows the filename so they know it took.

Where the file lands:
The attachment is saved with the submission. Open the submission (Intake -> Submissions) and the attachment shows as a download link with the filename — click to open or save it. For image attachments, the same platform image-moderation that covers every uploaded image applies: an image still being reviewed is held until it clears, so nothing unmoderated is shown. Non-image documents (a PDF, a spreadsheet) are available to download immediately.

Attachments are review-only — the file is there for a human to open from the submission, not routed into another module as structured data.
