Record an observation — vitals, notes, behaviors

An observation is anything you noticed: a vital sign you took, a behavior you saw, food intake, mood, pain, sleep. The Log tab is the running stream.

To record:
1. Log tab → "+ Observation"
2. Pick a type (vitals, mood, intake, behavior, pain, sleep, general note) — drives which fields show.
3. Fill the relevant fields. Vitals get structured slots (temp, BP, HR, SpO2, weight); others are free-form.
4. Save

Every observation is timestamped and tagged with who logged it. Trend views (Today tab) summarize the last 24 hours.

Hand-off context:
- The Log is the surest cross-shift record. When the next caregiver opens Care, they see the last handoff at the top, then the most recent observations below.
- Vital trends matter more than single readings — log consistently and the pattern shows up.
