Write a shift handoff

A handoff is the short narrative one caregiver leaves for the next. It's the most important thing in Care.

To write one:
1. Log tab → "+ Handoff"
2. Free-form narrative. We don't enforce a structure but SBAR works well:
- **Situation** — what's the current state in one sentence
- **Background** — what changed during your shift
- **Assessment** — your read on what's going on
- **Recommendation** — what the next caregiver should watch / do
3. Save

The handoff appears in Today as "Last handoff: X minutes ago" and at the top of the Log timeline. Every circle member is notified when a new handoff posts.

Why this matters more than a chat: a handoff is permanent, attributable, and lives next to the observations + med log that justify it. Chat threads scatter; the handoff is the single source of truth for "what does the next person need to know."
